Takahashi Supplemental Information

This year we are using an on-line submission for your supplemental entry. There will be no paper form.
Instructions
1. Watch training video by clicking HERE
2. Complete the form by clicking the Open Supplemental Entry Form below
3. Do NOT submit an entry for yourself and your Takahashi partner, that happens automatically
4. You can submit multiple forms
5. The Submitter is responsible for the supplemental payment at the Friday meeting
6. If you find you need a submission to be changed or deleted please click here to send an email stating what you want
7. Please communicate with your fellow members to coordinate who will submit so to avoid duplicate entries
8. The table below will update every 5 minutes, don’t panic if your entry doesn't immediately appear
9. Use the Find feature (CNTL F) in your Internet browser to find names
10. The table below is sorted by the Name of Submitter and re-sorts every 5 minutes
11. The submissions AND CASH PAYMENTS must be completed by 7:45PM the Friday before the Takahashi
12. Failure to make payment by 7:45PM the Friday before the Takahashi will result in the removal of entries
